A Toyota Technical Service Bulletin first issued in 2008 and revised through 2013 documents the process behind that offer: a customer visits a Toyota dealership to be measured and complete a Seat Belt Extender Worksheet; the dealership determines the correct part number, places the order, and reviews written Owner Instructions with the customer. Dealers are instructed not to stock extenders in advance, since each one is fitted to the individual rather than kept as a generic part. The bulletin's model application table also assigns different extender types to different models and seat styles (for example, a bench-seat Tacoma uses a different type than its bucket-seat version). The extender itself, not just the fitting process, varies by application.
Toyota's program treats seat belt extender selection as application-specific rather than universal. The appropriate extender is determined for the individual, vehicle, seating position, and required length.
The bulletin includes the Customer Information Label affixed to each extender at the time of fitting:

Toyota's own stated rationale for the restriction is reduced seat belt restraint effectiveness outside the fitted application. The bulletin does not describe underlying test data or engineering studies beyond that stated reasoning.
Toyota's current manuals carry the same usage limits described in that bulletin: do not use the extender if the seat belt can be fastened without it, and never use it when installing a child restraint system. Toyota's published program shows that the company accommodates seat belt extenders within its restraint systems when they are selected and used according to defined application-specific requirements.

Two separate Highlander owners report contacting both Toyota directly and their local dealership seeking an extender for a middle-row center seat and being unable to obtain one; one was issued a window-seat extender despite explaining the actual seating position. Toyota's program addresses a specific, well-defined need — insufficient length at a supported seating position — and reported cases such as these illustrate circumstances its available offering may not resolve.
